Coined by Gabriel Krieshok after eight years working within Washington, DC government and nonprofit bureaucracies. The term captures a specific institutional failure: organizations that treat routine, recurring transactions as though they were unprecedented, forcing participants to navigate each encounter as if no one had ever done it before. The pattern reflects a broader problem of institutional knowledge loss, where processes are never codified and departing staff take operational memory with them.
